Scope of the position

We are seeking a bilingual and culturally competent Armenian and English speaking Care Coordinator to provide case management, mental health and/or substance abuse services, crisis intervention and linkage to community resources for individuals with a history of persistent mental illness and/or substance abuse; ensures participants receive the maximum benefit from all interventions.Ensuring a safe, nurturing environment for clients that fosters a sense of hope, dignity and direction.


Essential Functions

Depending upon specific program assignment and experience level, the Care Coordinator B/L I may perform any or all of the following duties:


  • Assists program participants to locate and utilize community support groups; establish and maintain housing; access mental and medical care, rehabilitation programs and drug/alcohol services; increase social contacts and engage in constructive activities.
  • Performs functional assessments; determines case management needs of clients and monitors progress as needed.

  • Acts as client advocate interfacing with housing and support agencies including property owners, support groups, Social Security, Medicare/MediCal and other; ensures clients receive all benefits for which they are entitled.
  • Assists clients in learning skills to cope with mental illness and/or substance abuse.

  • Provides crisis intervention and facilitates conflict resolution among the program participants as needed; assists clients in problem-solving life situations.
  • Interfaces with clinical staff (including psychiatrist) in coordinating client care.
  • Maintains documentation of client services; ensures compliance with Agency policies and procedures and County and other agencies’ standards and guidelines.
  • Develops and maintains skills through seminars, workshops, or other forms of training/education related to adults with mental illness.
  • Attends and participates in supervision, staff, case conference and other Agency meetings, as well as community agency meetings.  May transport clients to appointments, etc.



Required Qualifications

  • Bachelors Degree in related field preferred.
  • Must be able to provide case managenent services in both English and Armenian.
  • 1-2 years experience working with individuals with a history of mental illness in a social service/mental health environment.
  • Knowledge of case management, crisis intervention techniques, community resources, and the ability to match clients with a history of mental illness to community support resources.
  • Proven comprehension of the recovery model desired.
  • The ability to provide services in a language other than English preferred.
  • Knowledge of program HIPAA, DMH, policies and documentation guidelines, licensing, and other state/federal regulations relating to service programs for adults with a history of mental illness.
  • Demonstrated advocacy skills.
  • Strong written and oral communication and PC skills.
  • Current California drivers license and a driving record acceptable to the Agency's insurance carrier.
