  • Providing case management, mental health and/or substance abuse services, crisis intervention and linkage to community resources for individuals with a history of substance abuse and/or persistent mental illness; ensures participants receive the maximum benefit from all interventions.
  • Coordinates care between the Agency’s residential program and other community substance use treatment programs.
  • Ensures a safe, nurturing environment for clients that fosters a sense of hope, dignity and direction.



Primary Duties



  • Provides linkage, care coordination and discharge planning for clients in Didi Hirsch’s substance use residential treatment setting to other community substance use treatment programs.
  • Collaborates closely with residential treatment staff and the staff of other community organizations, related to treatment and discharge planning and coordination of care.
  • Assists program participants to locate and utilize community support groups; establish and maintain housing; access mental and medical care, rehabilitation programs and drug/alcohol services; increase social contacts and engage in constructive activities.
  • Perform client intakes and functional assessments; present functional assessments and diagnostic impressions to staff; determines needs of clients and monitors progress as needed.
  • Acts as client advocate interfacing with housing and support agencies including property owners, support groups, Social Security, Medicare/Medi-Cal and other; ensures clients receive all benefits for which they are entitled.
  • Provides individual/group supportive counseling and psycho-education to assist clients in coping with mental illness and/or substance abuse.
  • Provides crisis intervention and facilitates conflict resolution among the program participants as needed; assists clients in problem-solving life situations.
  • Interfaces with clinical staff (including psychiatrist) in coordinating client care.
  • Participates in providing on -call coverage as needed.
  • Maintains documentation of client services; ensures compliance with Agency policies and procedures and County and other agencies’ standards and guidelines.
  • Develops and maintains skills through seminars, workshops, or other forms of training/education related to adults with mental illness.
  • Attends and participates in supervision, staff, case conference and other Agency meetings, as well as community agency meetings.  May transport clients to appointments, etc.

Required Qualifications


  • A basic working knowledge of case management, crisis intervention techniques, community resources, and the ability to match clients with a history of mental illness and/or substance abuse to community support resources.
  • A working knowledge of program, division and Agency policies and procedures, HIPAA, Department of Mental Health, policies and documentation guidelines, licensing, and other state/federal regulations relating to service programs for adults with a history of mental illness and/or substance abuse
  • Basic advocacy skills.        
  • The ability to present ideas, information, and viewpoints clearly, both verbally and in writing
  • Personal computer skills including word processing software
  • The analytical skills, experience and good judgment to make decisions
  • A commitment to team objectives and Didi Hirsch philosophies
  • Ability to adapt to changing needs by acquiring new skills and knowledge
  • Current California driver’s license, proof of insurance and a driving record acceptable to the Agency’s insurance carrier


These requirements are typically met by a combination of education and experience that includes a bachelor’s degree in human services or related subject and experience working in a social service/mental health environment, preferably with mental health and/or substance abuse clients.  For Substance Abuse programs, a degree may be substituted for experience, however, CAADAC or CAADE registration or certification is required, with or without a degree.
