Scope of the Position

Care Coordinators are responsible for providing comprehensive case management services for Children in connection with mental health services and linkage to community resources.

 

 

Essential Function

  • Conducts/facilitates individual and group counseling and provides routine case management including assessment, intake, education, crisis intervention, and exit interviews for children adolescents with a history of mental illness and/or substance abuse.
  • Interfaces with such agencies as the DMH, criminal justice services, Public Health and other related community agencies and resources to give and receive information relative to clients.
  • Will coordinates and run the Diaper disbursement, and all other donation distribution.
  • Maintains documentation of all counseling sessions and other services; ensures compliance with Agency policies and procedures, contracts and other regulatory agencies.
  • Supervises program participants with medication compliance.
  • Provides crisis intervention and facilitates conflict resolution among the program participants.
  • Prepares education materials for staff training and presentations.
  • Develops and maintains counselor skills through seminars, workshops, or other forms of training/education.
  • Actively participates in all team and staff meetings, community meetings and supervision/consultation.

 

Requirements

 

Required Qualification

  • Bachelors Degree in related field and 1-2 years experience working with individuals with a history of mental illness in a social service/mental health environment.
  • Candidates must be Bilingual English/Spanish and able to Speak, Read, and Write in both English and Spanish.
  • Knowledge of case management, crisis intervention techniques, community resources, and the ability to match clients with a history of mental illness to community support resources.
  • Knowledge of program HIPAA, DMH, policies and documentation guidelines, licensing, and other state/federal regulations relating to service programs for adults with a history of mental illness.
  • Demonstrated advocacy skills.
  • Strong written and oral communication and PC skills.
  • Current California driver’s license and a driving record acceptable to the Agency's insurance carrier.
